Would it be possible to create a Karma top list? that could sit at the bottom of the index page.

Typically for something like this, you'd need to use a database to store said data so it's easily accessible. It can however, be possible with the database project I've been working on. I already have something installed on this forum to store data based on the user, which could be used for plugins. I've not utilized it yet, it's just something I setup when I first started doing it.
We could compare the data in the reputation column.

Yes I think so but something that would only show the name of the member and their Karma count ie.
Member1(20) Memebr2(31 Member3(345) and so on.
